GPAQ Practice Survey

What is this Survey for?

The survey allows patients to express their opinion on the practice and its services. This survey does not include individual surveys of GPs or Practice Nurses.

How is it administered?

Questionnaires are allocated to all patients of the practice and returned via the practice.  We will send you the report within 2-4 weeks of our receiving the last questionnaire.
