What is this Survey for?

We offer surveys of Individual GPs and Practice Nurses, which can be combined with a Practice Report for a full picture of the surgery’s performance.

Why use this Survey?

These are individual personalised reports which are ideal for those wishing to collect data for revalidation folders, appraisals and PDPs.

Who can use this Questionnaire?

This questionnaire can be used by nurses and nurse practitioners, although they do have their own Nurse GPAQ Survey.

How is it administered?

Questionnaires are allocated to all patients of the practice and returned via freepost.  We will then send you the complied reports within two –four weeks of the last questionnaire being received.
