GPAQ Practice Survey

The ideal solution for practices, allowing patients to express their opinion on the practice and its services.

The Methodology

• You will receive the appropriate number of questionnaires (depending on the size of your practice). 
• Patients complete the questionnaire and return the completed surveys to the practice.
• The completed forms are returned to InTime Surveys via recorded delivery.
• When sufficient questionnaires have been returned InTime Survey will capture the data and produce the analysis.
• We send you reports within 2-4 weeks of our receiving the last questionnaire.
