Individual GPAQ

Individual personalised reports which not only can be used to claim the points required for QOF payments but which are ideal for those wishing to collect data for revalidation folders.

This questionnaire can be used by nurses and nurse practitioners, although they do have their own questionnaire design.

The Methodology

• Individual boxes containing 75 questionnaire forms are allocated to each member taking part. 
• Questionnaires are given to all patients of the practice (excluding temporary residents and those under 16 years old) attending the surgery from the day the survey starts until all the questionnaires have been given out.
• Patients complete the questionnaire after their consultation, and return the completed surveys either via the surgery or via Freepost Envelope to InTime Surveys.
• InTime Surveys monitor the returned questionnaires and start processing the data immediately. As soon as sufficient questionnaires have been received (50 per GP) for every GP in the practice we will produce the report.
• We send you reports within two weeks of the last questionnaire being received.
• Practice reports can be produced by combining the individual data.
